FIU: Cellular Phone Allowance Policy

Detailed Instructions

Cellular Phone Transfer of Service Step-by-Step Instructions (Process Flow Diagram)

  1. Complete the Employee Cellular Phone Allowance Agreement. This form is located in the Policies & Forms section of this website. Complete, print, and submit the form to your manager or department head for approval. Make sure you gather all necessary signatures. Once all signatures have been obtained the form should be returned to the employee.

  2. Decide which cellular service provider you want. Once you have received approval for the allowance, see the “Cellular Providers” link on this website and decide which cellular service provider is right for you.

  3. Transfer FIU cellular service over to your name.

a. If you want to use AT&T as your cellular service provider:

i. Follow the Transfer of Service Instructions for AT&T located on the “Cellular Providers” section of this website.

ii. When you are done, update the Employee Cellular Phone Allowance form to reflect your current personal cellular information and submit the form for processing to the Division of IT's Service Managment Group in PC 538.
