FIU: Cellular Phone Allowance Policy

Summary Instructions

To comply with IRS regulations and to reduce costs, a new Cellular Phone Policy has been developed and will become effective July 1, 2008. All employees with an FIU-issued cellular phone must comply with this policy and must do one of the following by July 1, 2008:

OPTION 1
If you do not qualify for the cellular phone allowance or if you are no longer required to have a University cell phone, you will need to disconnect your service by July 1, 2008. To disconnect your service, please visit our website http://uts.fiu.edu or call the Support Center at 305-348-2284 to initiate a service request.

OPTION 2
If you qualify for a cellular phone allowance, please follow the steps below.

  1. Complete the Employee Cellular Phone Allowance Agreement. This form is located in the “Policies & Forms” section of cellpolicy.fiu.edu. Complete, print, and obtain all applicable signatures.

  2. Decide which cellular service provider you want. Once you have received approval for the allowance, see the “Cellular Providers” link on this website and decide which cellular service provider is right for you.

  3. Transfer FIU cellular service over to your name.
    • If you want to use AT&T as your cellular service provider, please follow the AT&T Transfer of Service Instructions located in the http://cellpolicy.fiu.edu website under “cellular providers
      1. When you have completed the above steps, update the Employee Cellular Phone Allowance form to reflect your current personal cellular information and submit the form for processing to the Division of IT's Service Management Group in PC 538.
      2. If you decide to upgrade your device, the existing FIU-issued AT&T device (i.e. your current cellular phone) must be returned to your department, along with any accessories.
    • If you want to obtain cellular services from another carrier:
      1. Follow the transfer of service instructions for any of the other carriers listed under the “Cellular Providers” section. (Note: you are not just limited to the carriers listed on this website). Return the FIU-issued AT&T device (i.e. your current cellular phone) to your department, along with any accessories.

        You may need to provide the vendor with the following information:
        • FIU billing account number with AT&T: 820027338
        • FIU tax ID #: 650177616
        • First and Last Name
        • Social Security Number (there is a credit check)
        • Driver’s License Number and Expiration Date
        • Date of Birth
        • Home and Work Phone Numbers
        • Billing Address (possible physical address if PO Box is main)

  4. Update the Employee Cellular Phone Allowance form to reflect your current personal cellular information and submit the form for processing to the Division of IT's Service Management Group in PC 538.

Note: Most vendors offer two types of Blackberry service plans (Enterprise and Personal). To synchronize your device with FIU’s Blackberry Enterprise Server, you must choose the vendor’s Blackberry Enterprise plan. If you have been migrated to the FIUmail system (FIU’s Microsoft Exchange Server) you may access your FIUmail account with a Windows Mobile device. Click here for instructions.
